Intake Process:

Contact agency directly to obtain intake screening and schedule an appointment

Program Fees:

$20 for a regular extraction

$50 for wisdom teeth extraction

Eligibility:

Serves children, adolescents and adults needing acute dental services, who meet income guidelines

Income verification required, must bring paycheck stub and picture ID
